Initial Assessments Free
The initial assessment helps me understand your situation, your needs, and what support would bring you the most peace of mind. We review health history, medications, safety, and current concerns so care can be tailored to you.

Best for occasional support, doctor visits, or one-time needs.
Includes:
In-home nursing assessment
Medication and symptom review
Observation of mobility, cognition, and safety
Identification of potential risks or concerns
Verbal guidance on care priorities
“What to watch for” explanation
Follow-up recommendations
Optional coordination with providers, if needed
Additional time (if needed):
$90 per additional 30 minutes -
Flat Add-On Service
may include catheter care, tube feeding assistance, ostomy care, injections, and other procedures when medically appropriate
Remote Only
(Care coordination only through phone/email support)
Unlimited phone/email/text support during business hours (Tues–Fri 10am–6pm, Sat 1pm-6pm)
Scheduled video/call check-ins

Designed for higher-acuity needs, dementia care, or complex conditions.
Wound Care & Recovery

Up to 6 visits (≈3 visits per week)
Best for post-surgical wounds (Mohs, biopsies) or new wounds needing close monitoring. -
Up to 12 visits (≈3 visits per week)
Ideal for slow-healing wounds, pressure injuries, or venous ulcers -
Up to 18 visits (≈3 visits per week)
Designed for chronic wounds, high-risk clients, or adherence challenges. -
For clients receiving wound care who also want general/wellness ongoing care coordination between visits
(Includes remote care coordination)
